Dubois County Looking to Assess Children's Immunization Rates
The Dubois County Health Department is working on a project to assess overall child immunization rates in Dubois County.
In order to do so, the department is looking for parent’s feedback on their child’s immunization habits and services. They say it would be tremendously helpful if everyone could help push out the Immunization Community Survey link below.
This survey is an anonymous 5 question survey that can provide feedback to help us better assist our community’s needs. The link is also posted on our Dubois County Health Department Facebook page.
