Dubois County Health Department to Hold Late-Night Pfizer COVID-19 Vaccine Clinic for Children Aged 5-11

Local Sources - The Dubois County Health Department will be holding a special late-night clinic to accommodate the 5–11-year-old pediatric patients wanting the Pfizer Covid-19 vaccine. 

The clinic will be held at the Dubois County Health Department from 3:30-6:00 pm on Tuesday November 9, 2021. 

Anyone 5 and older is eligible to receive Pfizer Covid-19 vaccine. A parent needs to be present for anyone 5–18-years old. If this is not an option, you MUST complete the CONSENT FOR COVID-19 VACCINATION FORM attached below. A copy of a parent’s ID and insurance card must be included. 

DATE: Tuesday, November 9, 2021 
TIME: 3:30 P.M. – 6:00 P.M. (NO APPOINTMENT NEEDED) 
LOCATION: 1187 South Saint Charles Street, Jasper IN 47546. 

Please have ID and Insurance cards available. 

The drive-thru will NOT be open, so please come inside when you arrive.
